Job Description:
We are currently seeking an experienced Clinical MTM Pharmacist to join our Medication Therapy Management (MTM) team. This role focuses on improving patient outcomes through comprehensive medication reviews, patient consultations, and accurate clinical documentation.
Key Responsibilities:
- Conduct outbound calls to patients (approximately 100 calls per day).
- Perform Comprehensive Medication Reviews (CMRs) and Medication Therapy Management (MTM) services.
- Provide detailed consultations regarding statin therapy and medication adherence based on AHA guidelines.
- Review patient medication and diagnosis information and accurately document recommendations.
- Enter prescription, diagnosis, and clinical recommendation data into a web-based application.
- Ensure all documentation is completed accurately, efficiently, and in compliance with company standards.
- Participate in training sessions and ongoing development activities.
Required Qualifications:
- Active Florida Pharmacist License (please include a screenshot of your license at the top of your resume).
- Minimum of 4 years of Clinical MTM Pharmacist experience.
- Experience conducting medication reviews and comprehensive medication management.
- Strong understanding of Medicare Part D Star Measures, including SUPD, SPC, and CMR.
- Excellent verbal and written communication skills.
- Ability to speak, read, and write English fluently.
- Strong attention to detail and ability to maintain patient confidentiality.
- Comfortable working with technology and web-based applications.
