Florida State University's College of Medicine has been awarded more than $19 million through the Rural Health Transformation Program to strengthen healthcare access and outcomes in rural Florida. The funding will support six new projects focused on critical needs including behavioral health, preventive care, food insecurity, telehealth, at-home care and healthcare workforce development. Through partnerships with communities and healthcare organizations across Northwest and Southwest Florida, FSU is working to bring innovative, practical solutions directly to rural communities and help close longstanding gaps in care.

Responsibilities
  • Conduct health needs screenings and care navigation at every mobile unit stop. Will travel throughout Walton County attending mobile clinic events, visiting local clinics, interacting with community paramedicine providers, and ensuring patients are connected to the resources they require.
  • Initiate and track referrals to Community Paramedicine, Remote Patient Telemonitoring, Retail Clinic Services and fixed-site primary care providers.
  • Confirm appointment completion and document barriers to follow-through.
  • Connect patients to local health and human services resources including WIC, transportation assistance, and social support programs.

Qualifications
  • High school diploma/equivalency and two years of experience.
  • Valid FL or GA driver's license or the ability to obtain prior to hire.
  • Community Health Worker Certification (CHWC) must be achieved within the first year of employment.

Preferred Qualifications
  • Ability to adhere to ethical business practices, promote ethical behavior, and represent the University in appointed area as required.
  • Ability to demonstrate effective customer service skills and establish and maintain effective working relationships.
  • Ability to communicate effectively verbally and in writing and gather, interpret, report, and/or edit information.
  • Ability to perform accurately in a detail-oriented environment and prioritize, organize, and perform multiple work assignments simultaneously.
  • Ability to maintain accurate records and retrieve files from active or archive sources. Basic filing practices to include numeric and alphabetic systems.

Helpful

This role will be the primary accountability bridge across five Rural Health Transformation Program initiatives that form a network to expand access to preventive and primary care services for rural populations. The initiatives are (1) mobile health, (2) on-site pharmacy/retail clinic services, (3) community paramedicine, (4) remote patient telemonitoring and (5) health information exchange.

This position will be responsible for traveling throughout Walton County and will support Bundle 1 - Preventative Care & Care At Home Northwest region, which is defined as Bay, Calhoun, Escambia, Franklin, Gadsden, Gulf, Holmes, Jackson, Jefferson, Leon, Liberty, Okaloosa, Santa Rosa, Wakulla, Walton, Washington on the Rural Health Transformation Program Map .
